Proven Process

  1. Assessment: HBC begins with your Metabolic Assessments. These are required to establish a baseline and to quantify progress. How you are feeling is captured with qualitative discussions. Additional evaluation will come from your medical history, lifestyle factors and barriers.
  2. Goal Setting: Through a collaborative discussion we establish specific, measurable, attainable, relevant, and time-bound (SMART) goals. Your personal metabolic formula, needs and abilities guide your SMART goals. This involves evaluating medical history, lifestyle factors and personal goals. Understanding your barriers play a crucial role in the goal setting process. 
  3. Health Behavior Changes Strategies: We utilize motivational interviewing, cognitive-behavioral strategies and goal-setting frameworks to facilitate change. We help you identify and overcome barriers to change, develop coping strategies and build self-efficacy.
  4. Support and Accountability: We are your ongoing support through regular sessions, which can be face-to-face, over the phone or video. Together we track progress, review achievements, and make adjustments to the plan as needed.
  5. Education and Skill Building: We provide education on health topics relevant to your goals, such as nutrition, exercise, sleep, stress management and chronic disease management. Our goal is to help you build independent skills and habits that support long-term health changes.
